For your first office visit:

Please arrive 15 minutes early for your first appointment to complete the necessary paperwork.

Please download New Patient Packets to complete the forms and bring with you when you visit the office.

Bring to your appointment:

  • List of Current Medication(s)
  • Current Insurance Card
  • Completed New Patient Packet
  • Co-payment due at time of service
  • Referral from your physician (if applicable)
  • All X-rays, CT scans, MRI scans and other pertinent lab reports

If you are unable to keep your appointment, please call us at least 24 hours prior to your scheduled appointment time. $35 will be charged to No Show/No Call appointments.

If your insurance plan requires a referral from your primary care physician, please obtain a referral prior to your visit.

ORTHOPEDIC AND WELLNESS INSURANCE LIST

Some insurance plans require us to have a referral from your primary care physician before we can see you for an evaluation, office visit or procedure. The patient is responsible for knowing that a referral or authorization is required, obtaining a valid referral and bringing it to the scheduled appointment. Due to insurance requirements, patients arriving without a valid referral may need to be rescheduled. CO PAYMENTS, COINSURANCE AND DEDUCTIBLES Co payments, coinsurance, deductibles and any previous balances are due at check-in. We accept cash, American Express, Visa, Mastercard and Discover. Patients who anticipate difficulty in paying for balances are encouraged to contact our billing department. Payment plan arrangements will be made prior to the appointment and finalized at check-in for such balances. Our staff is available to assist you with any questions regarding your insurance company requirements or your account balance. Please call for insurance concerns.
