REFERRALS AND PRE-CERTIFICATION   

Some insurance plans require us to have a referral from your primary care physician before we can see you for an evaluation, office visit or procedure. The patient is responsible for knowing that a referral or authorization is required, obtaining a valid referral and bringing it to the scheduled appointment. Due to insurance requirements, patients arriving without a valid referral may need to be rescheduled.

 

CO PAYMENTS, COINSURANCE AND DEDUCTIBLES   

Co payments, coinsurance, deductibles and any previous balances are due at check-in. We accept cash, American Express, Visa, Mastercard and Discover. Patients who anticipate difficulty in paying for balences are encouraged to contact our billing department. Payment plan arrangements will be made prior to the appointment and finalized at check-in for such balances.
